VISION INSURANCE COVERAGE

The following description of the Eye Care Network Program has been included in this brochure for the convenience of the student. The Vision Coverage is in no way related to the benefits, limitations and exclusions of, and in no way affects the coverage provided by, the Student Health Insurance Plan described herein.

As part of this plan, the Insured has access to the Discount Vision Program offered through The Eye Care Network. This program entitles the Insured to receive a 20% discount off usual charges when using a participating provider. The 20% discount applies to the following eligible expenses: routine eye examinations, lenses, frames, contact lenses, plus cosmetic extras such as tints, coatings and photochromatic lenses. The discount does not apply to disposable or replacement contact lenses, eyewear repairs, promotional offers or medical/surgical treatment of the eyes. To receive the 20% discount, the Insured should present his or her ID card to any Discount Vision Program participating provider at the time of service. Only those providers listed with the Discount Vision Program will offer the 20% discount. An additional offer to buy-up on the Vision Plan and increase your vision benefits may be available.
